Cheat Mode (NTSC):
Pause game play, then hold L2 and press Triangle, Circle(2), Triangle, Square, Circle, Left, Circle, Up, Down.
Extended Cheat Mode (NTSC):
Pause game play, then hold L2 and press Square, Triangle, Circle, Up, Triangle(2), Right, Circle, Left(2), Square, Triangle, Right, Circle, Left(2), Square, Triangle, Circle, Down, Circle(2), Right.
I wouldn't call the "Level Select" cheat floating around the Internet that. It's just a way for you to move around the level quicker.
Teleportation:
Pause game play and enable the "Extended cheat mode" code. Go to the camera option and select plugin type "8". Press Start to resume game play. A sword and text will appear on the screen. The functions of the camera are as follows: L1 to zoom out; L2 to scroll down; Triangle/R1 to zoom in; R2 to scroll up. Pressing X will switch between "Focused" and "Free-motion" modes. Use the D-pad to point the sword at a destination, then pause game play. Go to the camera option again and press X to change the plugin type to "Clear". Press Start to resume game play at your selected destination.
