It seems Form Filler messed up some of the items.
About the duplicate entries I don't know why I didn't see them. I added a bunch of codes in a few hours so its easy to commit mistakes ^^.

Ok, you know what? Those Deep Dungeon 3 item modifiers are a mess. Let me fix it up a bit. Remove all the item modifiers (the last 4 entries) and replace with this sole one:

Value 02 caused the character to be invisible as well, changed it to 03 and it worked properly. Was this due to the emulator I'm using or something? I'm using Nestopia.

In mine it was the other way around. The values just turns the flickering on or off. Just change them if your character is invisible. I've seen it happen with a lot of Invincibility codes.
