The lives counter only goes to 4 at the most, so it just keeps counting down to 0 and then resetting to 4 and counting down to 0 and repeating. It's not a bug.
Level Specific Codes
They only work on these level for some reason.
(Junkyard, Minion's Stadium, Prison Ship, Snowy Roads, Drive-In Movie, Abandoned Skyscrapers, and Warhawk's Rooftop)
No matter what I did or how many nearby addresses I checked, I couldn't get them to work on other levels. They work in 1 & 2 player story mode and 1 player endurance. Player 2 codes affect the CPU enemy in 1 player endurance mode.

It's a little hard to look around with 2 players, but it's kind of cool. I don't know why they don't have this in the game. The CPU is even good at flying.
