By analyzing the way the cheat menu is expanded in the Mega Drive version of The Smurfs, I have figured out how to do the same in the Mega CD version. Look for address 1514A and replace value 00 with 08, or use Game Genie code BBJT-CACL. This is especially useful in the Mega CD version, because for some reason the regular cheat menu of the CD version was simplified and you could only select your level, but not setting your energy like you could do in the regular cheat menu of the cartridge version. This restores that function, together with other debug functionalities that are also added this way.
During this process, I also figured out how to do the same in the Brazilian Mega Drive version (the only official NTSC version of this game), which is not exactly the same as it's done on the European Mega Drive version. You have to look for address 7A092 and replace value 00 with 08, or use Game Genie code BCTA-RAEW.
