They'll change your custom title to what? I'm not sure why there aren't any codes for it, but based on this:

Phantasy Star IV is also part of the Sega Genesis Collection on the PlayStation 2 and PlayStation Portable and on Sonic's Ultimate Genesis Collection for Xbox 360 and PlayStation 3.

there shouldn't be a separate entry for it; codes for this game should be placed in the Sega Genesis Collection.
