Can't add codes to Genesis section

Everything seems to work fine when adding Game Genie codes to the Genesis section, but when I click on "Submit", the codes are not added.

Same here, tried to add a code for the N64 section.
Also, there was an error when trying to upload an attachment (.ips).

Fixed code adding. Added the master bit to the form.
